　　Recently,there have been four large coal mine accidents in Sichuan,Guizhou,Hebei and other places,causing a total of 21 deaths.The impact is bad,the lessons are profound,and the coal mine safety production situation is grim.In order to draw lessons from accidents,to curb the trend of coal mine accidents,and to effectively strengthen coal mine safety production,the relevant situation is now reported as follows:

　　1.Basic situation of four major accidents

　　(1)"7·28"large gas explosion accident in Houxin Coal Industry Co.,Ltd.,Wangcang County,Guangyuan City,Sichuan Province.

　　On July 28,2019,a gas explosion occurred in Houxin Coal Industry Co.,Ltd.,Wangcang County,Guangyuan City,Sichuan Province,causing 3 deaths and 2 injuries.The mine is a private enterprise with an approved production capacity of 90,000 tons per year and is a low-gas mine.

　　According to preliminary analysis,the main ventilation system of the mine is not well ventilated due to collapse of the roadway.Local coal fans are used for the ventilation of the roadway coal mining face.Gas is accumulated in the breezeless operation.Detonate 2#to accumulate gas in the upper part of standing eye.The main problems exposed by the accident were as follows:First,the mine was shut down on June 12 due to hidden dangers such as the collapse of the safety exit on the working face and multiple repairs in the return air lane.Suspended,but the mine was illegally produced in the name of maintenance and rectification of the working face.The second is to adopt the“lane-type coal mining”process that is expressly prohibited by the state.The ventilation management is chaotic,and a local fan is used to supply air to the three eyelets at the same time.The third is that no safety monitoring facilities have been installed at the accident face,and operation procedures and safety technical measures have not been prepared.Fourth,the gas inspection and blasting management were chaotic.The systems of"one shot and three inspections"and"three-person chain blasting"were not implemented.The personnel were not evacuated to a safe place when firing.Fifth,the mine was suspected of concealing or late reporting accidents and blindly organized rescue operations.

　　(2)"7·29"large coal and gas outburst accident at Longwo Coal Mine,Guizhou Zheshang Mining Group Co.,Ltd.,Xiuwen County,Guiyang City,Guizhou Province.

　　On July 29,2019,a coal and gas outburst accident occurred in the Longwo Coal Mine of Guizhou Zheshang Mining Group Co.,Ltd.,Liuguang Town,Xiuwen County,Guiyang City,Guizhou Province,causing 4 deaths and 2 injuries.After the accident,the mine concealed the accident and later verified it by the people's report.The mine is a private enterprise with an approved production capacity of 150,000 tons per year.It is located in a coal and gas outburst mine area.The gas grade identification result is a low gas mine,and it is identified that there is no danger of coal and gas outburst in the area above+977.3 meters.

　　According to preliminary analysis,the mine arranged illegally to arrange coal production in the Dongxiashan coal mining face outside the scope of outburst risk identification.No anti-outburst measures were taken.The coal shearer cut coal to induce coal and gas outburst.The main problems exposed by the accident are:First,the coal mining face is illegally arranged and is suspected of ultra-deep cross-border mining.The production was illegally organized in such a way that the concealed area was not pictured,the construction was temporarily sealed,the physical records of entry and exit wells were recorded,and the personnel in the concealed area did not carry the position detection identification card.The second is to fail to carry out the prevention of coal and gas outbursts as required.The coal mining face is arranged outside the scope of outburst hazard identification,and the gas parameters have not been measured,nor any anti-outburst measures have been taken.The third is that the accident working surface has not prepared operating procedures,installed no safety monitoring equipment,and has no self-rescue facilities for compressed air.Fourth,the ventilation management is chaotic,the ventilation facilities are unqualified,and the air volume at the coal mining face in the eastward mountain is insufficient.Fifth,they deliberately concealed the accident.After the accident,they did not report the accident to the relevant departments and transferred the corpses.When the relevant government departments received the masses to report to the mine for verification,they lied to the accident site and sealed the accident area,and organized related personnel to confess.

　　(3)"7·31"large gas explosion accident in Dashu Coal Mine,Guizhou New Southwest Mining Co.,Ltd.,Liangyan Town,Qixingguan District,Bijie City,Guizhou Province.

　　On July 31,2019,a gas explosion occurred in Dashu Coal Mine,Guizhou New Southwest Mining Co.,Ltd.,Liangyan Town,Qixingguan District,Bijie City,Guizhou Province,causing 7 deaths and 1 injury.The mine is a private enterprise with an approved production capacity of 150,000 tons/year.It is a low-gas mine.In 2015,it obtained approval for a merger and reorganization of 450,000 tons/year.

　　According to preliminary analysis,the 17301 coal mining face of the mine was flooded due to water inflow in the transportation lane,and the full negative pressure ventilation system of the working face was blocked.On the working face,gas accumulates in the roadway,and the gas explosion caused by the fire source.The main problems exposed by the accident are:First,illegal operation,continuous heavy rainfall led to increased water inrush in the transportation lane of the 17301 coal mining face of the mine,and the mine failed to take timely water damage prevention measures,causing the face and some system roadways to be flooded.Instead of fully organizing drainage,local fans were used to supply air to the return air lane,and construction of the uphill digging face in the return air lane was undertaken to mine coal illegally.The second is that the management of ventilation gas is not in place,the safety monitoring system is not running,the operator does not carry a portable methane detection alarm,and takes risks without checking the gas.The third is blind rescue.After the accident,the coal mine enterprise failed to report to the relevant departments in time,and the mine chief and chief engineer organized the rescue themselves,which almost caused a secondary accident.Fourth,the mine has been organizing production in the name of a 150,000-ton/year production mine since obtaining the approval of 450,000-ton/year merger and reorganization in 2015,and has not actually carried out merger and reorganization related work.Fifth,the higher-level coal mine company is an attached shell company,and has not actually held a controlling stake in its affiliated coal mine,and has not effectively supervised and managed the coal mine.

　　(4)Hebei Province Kailuan Group Tangshan Mining Company"8·2"large impact ground pressure accident.

　　On August 2,2019,a rockburst accident occurred in Tangshan Mining Company,Kailuan Group,Hebei Province,killing 7 people.The mine's approved production capacity is 4.2 million tons per year,which is a high-gas,rock burst mine.

　　According to preliminary analysis,rock burst occurred in the F5010 liaison roadway(mining depth of nearly 800 meters)in the coal pillar area ofthe mine,causing the roof of the roadway and the surrounding two roadways to fall.The detailed cause of the accident is for further investigation.

　　The above-mentioned 4 accidents once again reflected that some regions and enterprises could not stick to the red line awareness,and were driven by interests to take risks.The heads of enterprises risked organizing illegal productions,resulting in accidents.It also reflected the impact of ground pressure,coal and gas outbursts,and water damage.Major safety risks.
